What to Bring

To streamline your DOT physical and ensure we have all the information needed for FMCSA compliance, please bring the following:

  • Valid photo ID (driver's license)
  • Current CDL if renewing
  • List of all current medications
  • Glasses or contacts if you wear them
  • Hearing aids if you use them
  • Any medical records for existing conditions (diabetes, heart conditions, sleep apnea documentation)
  • CPAP compliance report if applicable
